The Tombstone Imperative
I ordered Andrew Wier's Tombstone Imperitive from Amazon UK (for about 7 pounds, whatever that converts to). I can't recommend it strongly enough for anyone interested in Air Safety. I haven't finished it yet, but I can tell you that Weir is an excellent writer. He does an excellent job explaining complex technical, statistical and regulatory topics in very readable language.

He quotes one anonymous US Aviation Offical as saying, "We regulate by counting tombstones." The 'Tombstone Imperitive' means this: Safety changes don't get made until enough people have been killed to warrant them.

A sad state of affairs!
